TYPE 2 DIABETES SUPPORT

To join the programme, you must be:
- Aged 18 or over
- Diagnosed with Type 2 diabetes in the last three years
- Motivated to make lifestyle changes
- Be comfortable using a Smartphone app for lifestyle support
- Registered with a GP practice in Angus, Dundee, or Perth and Kinross
Here's what you can expect:
- A FREE 12 week diabetes education and lifestyle support programme
- One-on-one health coaching tailored to your goals and the option to join a peer support group
- Access to the Oviva app, to track your progress and message your health coach
- Educational resources covering key aspects of weight management and Type 2 diabetes

Getting Started: What to Expect
After enrolling, an Oviva team member will contact you within two working days to gather information about your goals. Once your registration is complete, you'll be paired with a health coach who will offer support through either the Oviva Smartphone app, or over the telephone.
WEEKS 0-12
During the 12-week programme, you'll receive regular coaching (individual or group) to help you set and achieve personalised health goals. You'll have access to educational resources on Type 2 diabetes, including articles, videos, podcasts, live webinars, and a peer support group.
Beyond the Programme
After the programme finishes you will have continued access to the Oviva app's resources and features, empowering you to maintain your healthy lifestyle changes and manage your diabetes long-term.
